Equipment like walkers, scooters, bedside commodes, respiratory equipment, wheelchairs etc. are available. If you are in need of equipment, complete an application and submit it to the Ilva Romano Memorial Fund for approval.

Eligibility Requirements

  

Patient applicants must be a resident of Miami Dade, Broward, Monroe, or Palm Beach counties, Florida.
Patient applicants must disclose personal financial information, including a copy of TAX return information.
Patient applicants must provide documentation that all other avenues of funding have been exhausted.
Patient applicants must file for an Explanation of Benefit (EOB) with the insurance company.
Patient applicants need to provide complete information on services or equipment for which she/he is applying, and the associated cost.
Each service or piece of equipment assistance request requires that a separate application form be completed and submitted.

EQUIPMENT AVAILABLE FROM ILVA ROMANO FOUNDATION

If patients are interested in any of these devices, please fill out the same application form.  MS WORD (version) 

    1. Respiratory Equipment: Bi-PaP, In-Exufflator, Ventilator LP10
    2. Walking Devices: Adjustable cane, Folding walkerette, Mobile sit-down walker, Winnie walker, Arms rest standing walker with wheels.
    3. Transfer Aids: Lift Chairs (2), Swivel seat cushion, Multifold ramps with Carrying case.
    4. Mobility Devices: Guardian Transport Wheelchair for small frame person.
    5. We also have an EZ-Bathe; a vinyl tub to be used with patient in bed, comes with a hose and a wet-and-dry vacuum.
